chmod +x wrtbwmon

also from putty enter the command:
/bin/sh /jffs/wrtbwmon setup
(of course your path will vary)

then check in the /tmp/www/ folder to make sure the usage.html file is there. ...

Im assuming youve updated this line with the correct users.txt filename since i remember you said youd changed it
